Third Annual Dementia Capable Care Transitions Stakeholder Conference: February 26, 2020 at the Suncoast Hotel and Casino, Las Vegas

It is with great pleasure that I invite you to join in an innovative and much needed initiative to develop and implement an evidence-based care transitions model targeting the dementia population.    To our knowledge, the development of an evidence-based platform that integrates caregiver support and community-based services for this population is a first in the nation. The intention is to create a replicable model.  Nevada Senior Services is entering the third year of a 3 year grant from the Administration for Community Living. We also believe that this is the largest initiative of its kind awarded to a Nevada non-profit organization by ACL.

We initiated this project by creating a strong viable option for a hospital-to-home evidence-based platform.  We have found that partner in Bridge, a proven initiative of Rush University Medical Center (Chicago) and one of the original CMS Care Transitions demonstration sites.  We have organized a collaborative project team which includes: Bridge (Rush University Medical Center); Valley Health System; Sanford Center for Aging (evaluation); Comagine (HealthInsight); and, Cognitive Solutions (Mike Splaine, a leading consultant).  We have successfully implemented the initiative in the Valley Health System and with collaborating partners such as the Cleveland Clinic Lou Ruvo Center, first responders and other community agencies.

Addressing this critical need opens a door for families to receive critical support during their transition from hospital to home.  This is very important for families with over-stressed care givers and is a model for all chronic diseases across the lifespan.  We recognize that persons living alone with a dementia and persons with IDD represent special challenges to us in developing an effective model. We also recognize that this journey will raise important public policy questions that will need to be addressed.  Your participation would make a great contribution to a successful outcome.

The 2020 Stakeholder Conference will focus on Solutions and Sustainability.  We are pleased that Erin Long MSW of the Administration for Community Living (ACL) Office of Supportive and Caregiver Services will address the conference. Erin is our project officer and the leading voice in all things dementia at ACL.

You are a key community partner. The Conference will be of great value in solution development, setting the stage for future program direction and for identifying important issues for Nevada legislation.
